U.S. Social Security Administration – Regional Federal Benefits Unit Visit to Kingston & Montego Bay September 19th through September 23rd, 2022

The U.S. Embassy Kingston is pleased to announce that representatives from the Regional Federal Benefits Office in Santo Domingo will visit Jamaica to offer services for beneficiaries or individuals with questions about U.S. Social Security and other federal benefits.

Location:    US Embassy 142 Old Hope Road, Kingston 6, Jamaica, West Indies
Dates:         Monday 19th, 2022 – 8:00 A.M. to 12:00 and 1:00 P.M to 4:00 P.M
Tuesday 20th, 2022 – 8:00 A.M. to 12:00 and 1:00 P.M to 4:00 P.M

Montego Bay
Location:    US Consular Agency in Montego Bay, Whitter Village, Ironshore, Unit EU-1 (across from Burger King) Montego Bay, St James
Dates:         Wednesday 21st, 2022 –1:00 P.M to 4:00 P.M
Thursday 22nd, 2022– 8:00 A.M. to 12:00 and 1:00 P.M to 4:00 P.M
Friday 23rd, 2022– 8:00 A.M. to 12:00 and 1:00 P.M to 4:00 P.M

WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):

Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates

Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Change of Address for Social Security: Bring your current passport.

Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-1763.
