U.S. Social Security Administration (SSA) Services and other Federal Benefits

Location: U.S. Embassy Kingston, Consular Agency Montego Bay, Consular Agency Cayman Islands.

Event:  The U.S. Embassy in Kingston is pleased to announce that Representatives from the Regional Federal Benefits Unit will visit Jamaica and the Cayman Islands to offer services for beneficiaries or individuals with questions about U.S. Social Security and other federal benefits.

Please refer below to the schedule of services in each location:


Monday, June 10, 2019

Where:  Consular Agency Montego Bay
When:  9:00 am – 12:00 pm and 12:30 pm to 2:00 pm

Tuesday, June 11, and Wednesday, June 12, 2019

Where:  U.S. Embassy Kingston
When:  8:00 am – 12:00 pm and 1:00 pm – 3:30 pm

Thursday, June 13, and Friday, June 14, 2019

Where:  Consular Agency Cayman Islands
When:  Thursday, June 13 – 1:00 pm – 4:30 pm and Friday, June 14 – 8:00 am – 2pm

WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):

Proof of identification: passport, driver’s license, voter ID.

Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: birth certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates

SSA Proof of Life Study: In 2018, SSA mailed the questionnaire to beneficiaries receiving their own benefits whose social security number ended in 00-49 and beneficiaries over the age of 90. Please bring copy of your passport and a completed SSA-7162, if you still have your benefits suspended for this reason.

Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Change of Address for Social Security: Bring your current passport.

Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-2690.
